Carlos was admitted to Saint Francis Rehabilitation from Saint Vincent Hospital with significant shortness of breath and severe mobility limitations, initially able to walk only 3–5 feet and requiring assistance with basic self-care. To rebuild strength and independence, he participated in Physical and Occupational Therapy targeting ambulation, standing tolerance, transfers, and daily activities. In Physical Therapy, Carlos progressed through bed mobility, balance training, and gait training with a rolling walker, using the OmniCycle® to improve lower-extremity strength and endurance. Occupational Therapy focused on functional tasks such as dressing and household mobility, along with energy conservation strategies, core strengthening, and breathing techniques to better manage shortness of breath during activity; he also participated in therapeutic exercise and upper-body conditioning groups. By discharge, Carlos was walking over 100 feet without stopping and completing all ADLs with increased independence and confidence.

 

Daniel came to Saint Francis Rehab to regain independence and return home. With skilled therapy and the convenience of in-house dialysis, his care was seamless and supportive.

In Physical Therapy, Daniel worked on gait training, lower-body strength, stair navigation, and safe mobility indoors and outdoors—including household movement without assistive devices. In Occupational Therapy, he focused on strengthening, balance, and independence with bathing and dressing, practicing real-life tasks in the transitional mock apartment.

Daniel praised the team’s kindness and dedication, saying everyone went “above and beyond” and made him feel like family. With his hard work and the staff’s support, Daniel reached his goal of returning home with greater independence.

 

Christos was admitted to Saint Francis Rehab from Memorial Hospital with significant pain and mobility limitations. He required max assist x2 for sit-to-stand transfers and was unable to ambulate due to pain. During his stay, he underwent a laminectomy and then returned to Saint Francis Rehab to continue therapy with the goal of returning home.

Physical Therapy: Focused on balance training, gait training, and stair negotiation. The Omnicycle® was used to improve lower-extremity strength and endurance. Shortwave diathermy was provided for pre-operative pain management to improve comfort and participation.

Occupational Therapy: Addressed standing balance and transfer training to improve ADL performance. He also participated in group sessions to build endurance and functional strength.

At discharge, Christos was independent with transfers, able to ambulate household distances with a rolling walker, and completed all ADLs at a modified independent level.
