The right program for your needs

Our facility provides a full range of healthcare services, treatments and therapies to meet our resident's diverse medical needs.

Upon arrival, each resident receives a thorough evaluation to determine their physical, learning and emotional needs. Our staff of doctors, nurses, dietitians and social workers will then collaborate with the patient and his or her family to develop an individualized care plan to maintain and/or improve the quality of life. By utilizing an interdisciplinary team approach, combining a group of experienced professionals with the patient and family, we are able to design a care plan to offer the resident the best opportunity for a successful outcome.

Saint Francis Rehabilitation and Nursing Center is a 137 bed Skilled Nursing Facility located in the Grafton Hill area of Worcester. Our campus consists of 63 long-term care beds, including a 16 bed men’s only unit. The center has only private, semi-private and grand semi-private rooms. We have  five nursing units, 53 private rooms, two (2) small neighborhood Alzheimer’s Units totaling 37 secure beds, 35 Sub-Acute and a separate Adult Day Health program that can provide services for up to 60 participants per day. The skilled nursing facility has a beautiful Chapel and celebrates Mass six days a week.

During your hospital stay

When your loved one has an acute injury or surgery, there are many decisions to make during the hospital stay. One of them may be where to go for rehabilitation.

During your hospital stay, you will be assigned a case manager that assists you during your hospital stay and plays a vital role in assisting you with your discharge plan.

Typically, you will be given a list of choices for rehabilitation centers. Saint Francis employs a Registered Nurse as our Clinical Liaison and is able to meet with you in the hospital to discuss the type of care you need, your insurance information, and to review any rehabilitation questions you may have.

When possible, you or a loved one should visit our center so that you can make an informed decision.  The decision of where to receive post-acute care could be crucial to the outcome of your loved one’s recovery, and a return to their prior level of function.

One family member said that meeting with the Saint Francis Nurse Liaison in the hospital to discuss her dad’s nursing and therapy needs was imperative to choosing a facility for her dad’s rehab stay. She said, “I was able to have my questions and concerns answered, and to discuss the plan of care my dad would receive upon admission to the facility.”
